JOB SUMMARY: Responsible for assessing human responses to disease process, illness and/or injury.
Identifies health problems and provides restorative, supportive, preventive and specialized nursing
measures to individuals in their home by executing the medical regimen prescribed by a
licensed physician. (HH II.2c.1c, 1e)

Fully responsible for all actions as a licensed nurse and is accountable to patients for
the coordination and delivery of quality of care. Reports to designated immediate supervisor regarding work
planning and assignments.

Performs duties to provide the highest level of patient care and maximize patient satisfaction.

Reports directly to home care nurse manager.
MAJOR DUTIES AND RESPONSIBILITIES:

•1. Assesses patients by collecting and analyzing data to recognize patients health problems and perceptions. Interviews patients and/or caregivers to elicit all pertinent information including OASIS at appropriate time points as applicable.(HH II. 2c.1a)

*2. Plans nursing intervention designed to teach patient health management and maintenance to return patients to optimum level of health. Supports patients and patients' families with coping mechanisms when dealing with death and dying. Counsels patients and and patient’s family to meet identified needs.

*3. Initiates, implements, reviews and revises plan of care and nursing interventions to achieve desired outcomes. Monitors patients after implementation of care plan to assess effectiveness of care and patient response outcomes. Instructs all those involved in delivering care to ensure understanding and dealing with health problems. Notifies physicians of lab reports and significant changes in patients' conditions as warranted. (HH II.2c.1b, 1d, 1i, 1j)

*4. Makes appropriate referrals to other disciplines and programs/resources and coordinates overall care. Offers community resources to patients and families. (HH II.2c.1g, 1h)

*5. Maintains current, accurate and legible records on all patients.

*6.Promotes patient and/or family independence in managing their health care needs.

*7. Initiates telemonitoring system when appropriate.

*8. Initiates discharge planning at the initial home visit or as soon as feasible and continues discharge planning throughout patient’s home care stay. (HH II. 2c.1n)

*9. Provides direct supervision of licensed practical nurses and home health aides following supervision policies and procedures to assure standards of practice are being met. (HH II. 2c.1k.1l)

*10. Completes and submits all appropriate documentation within 48 hours. (HH II.2c.1f)

*11. Notifies the physician, other team members, and representatives from any involved community programs of discharge.

*12.Provides written and statistical reports or assignments and submits as required.

*13. Assumes responsibility for recertification of a medical plan of treatment and submits a summarized report to the physician when needed, but at least every 60 days, for review and signature.

*14. Collaborates and effectively communicates verbally and in writing with peers, patients, supervisors, physicians, and other professionals. Maintains good working relationship with peers, supervisors, and co-workers. Accepts people in a non-judgmental manner.

*15. Plans, coordinates and guides students and other health care team members who are assigned to work with the team.

*16. Actively participates in orientation program as necessary.

*17. Participates in in-service education. (HH II.2c.1m)

*18. Reports changes in patient status to supervisor as needed. Participates in team/care conferences.

*19. Functions as committee member when requested and contributes professional expertise
at assigned committees to provide continuous quality improvement. Participates in
community health programs.

*20. Maintains confidentiality regarding information on patients, staff and agency
matters.

*21. Assumes the legal responsibility of competency within the scope of the practice of
professional nursing.

*22. Distributes and uses patients' supplies prudently with primary care.

*23. Performs other duties as required or assigned by emergency or other operational
reasons for which the employee is qualified to perform.

SPECIFIC POSITION DUTIES:

COMPETENCIES AND SKILLS: Demonstrates strong interpersonal, oral and written communication skills.

Demonstrate the ability to comprehend, speak and write the English language

Demonstrates the ability to maintain accurate and legible progress notes.
EDUCATION AND/OR EXPERIENCE:
Graduate of an accredited school of nursing required.

Current license to practice as a registered nurse in Pennsylvania required.

Minimum of two years clinical experience required.

One year home health experience strongly preferred.

IV skills, venipuncture preferred.

Broad medical/surgical experience preferred.

SPECIALITY NURSE - EDUCATION AND/OR EXPERIENCE:
Maternal Child:

Minimum two years clinical experience and one year home health experience preferred.

Minimum one year obstetrical, maternal and/or newborn/neonatal experience preferred.

Pediatric Nurse:

Minimum two years clinical experience and one year home health experience preferred.

Minimum one year pediatric experience required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Duties involve occupational exposure to disease and illness including, but not limited to, Hepatitis B and TB viruses.

Requires HBV and TB vaccinations.

Must possess a current Pennsylvania driver’s license and have a vehicle available for work.

Must be available to work on call on a rotating basis.
