Under the general direction of the Program Manager but working collaboratively with the Senior Care Options representative(s), Case Manager plays an integral part of the case management functioning of the agency. Case Manager will develop a service plan based on their assessment of client needs, which directly impacts areas of concerns highlighted through the assessment. This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation.
Salary: $44,000- $52,000
Hours: 9:00am- 5:00pm (35 hour work week)
Travel: This position required up to 25% travel.
Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.
- Bachelor's degree or in the process of completing
- Bilingual a plus!
- 2+ years' experience in Human Services (internships) working in assisted living, senior centers, CNA
