Job description

Provides uninsured and underserved patients with non-clinical support and directreferrals to health and social support services in the community. The CHW works as part of an interdisciplinary team in the hospital setting. Primary focus is on finding medical homes for the emergency room patients that are utilizing the ED as the primary care facility.

  • Minimum of High School Diploma or GED, with Associate Degree preferred
  • Significant experience and knowledge of medical service delivery and community resources
  • Knowledge of the uninsured and underserved populations in area
  • Cultural Competence
  • The skill and knowledge competencies identified in the National Community Advisor Study, in June 1998, for Community Health Worker IIs. These include communication skills, service coordination skills, capacity building skills, interpersonal skills, capacity-building skills, advocacy skills, teaching skills, organizational skills, broad knowledge about the community health issues and social services
  • Basic computer knowledge
  • Ability to communicate effectively, both verbally and in writing
  • Bilingual ( Spanish preferred)
  • Skilled in gathering and reporting information
  • Strong organizational skills
  • Community Health Worker II Certification and or skills to qualify for certification (see core competencies) within 1 year of hire
  • CPR
