Job description

The Director of Care Management and Admission Transfer Center develops, implements and evaluates the strategic plan for the Hospital-wide Care Management process. Collaborates with all disciplines and departments to decrease length of stay, and optimize clinical and cost outcomes. Works to improve bed placement and hospital flow.



Responsibilities

  • Provides administrative leadership for the Care Management Department and the Admission Transfer Center.
  • Continually evaluates appropriate staffing levels, assignments, and deployment of the department. Facilitates departmental staff continuing education and professional development
  • Develops and administers the capital and operations budget.
  • Establishes and maintains effective relationships with physicians, hospital departments, and administrators.
  • Establishes effective relationships with external agencies. Facilitates transition of patients/families across the continuum by establishing relationships with appropriate providers and community services.
  • Evaluates and optimizes patient satisfaction in relationship to discharge planning functions. Continually assesses patient flow and looks for opportunities to improve.
  • Resolves conflicts related to discharge planning with patients/families, external agencies and payers.
  • Directs utilization management activities. Actively seeks opportunities to demonstrate leadership skills.
  • Promotes ethical behaviors as defined in the Hospital's Code Ethical Behaviors.
  • Manages the information system that supports decision making surrounding case management and social work.
  • Creates data priorities and report formats that display critical outcome factors.
  • Reviews findings with Senior Leadership.
  • Provides routine and timely feedback to the department staff regarding care management, bed placement and inpatient registration.
  • Provides routine and timely feedback to the appropriate medical department chair regarding findings.
  • Develops and/or contributes to an effective outcomes management process.
  • Establishes policies, procedures, and protocols for departmental functions.
  • Assures the Utilization Management and the Discharge Planning processes are current.
  • Creates processes to anticipate and address specific situations within the department, including but not limited to, long stay patients, post-acute referrals, denials, and termination of benefits.
  • Defines and operationalizes the department's role in care maps and protocols.
  • Assures compliance with applicable JCAHO standards for the Continuity of Care function.
  • Establishes, implements, and evaluates strategic plans for Care Management and the ATC.
  • Effectively communicates the plans throughout the organization.
  • Identifies organizational needs and concerns in payer-provider contracts and contributes to related negotiations.
  • Develops structures and processes to facilitate participation by Physician Advisors, Case Managers, Social Workers and other health professionals in multidisciplinary care planning, care delivery, and discharge planning.
  • Establishes effective communication systems and forums with third party payers.
  • On an annual basis, develops measurable, reasonable, and market-oriented goals for the departments that reflects those of the hospital and departments.
  • Identifies improvement opportunities within the departments and at the system level, and creates strategies to address them.

Qualifications

  • Candidate selected will possess clinical nursing experience of 5 years including current experience managing a department or supervisory position.
  • Additional knowledge and experience (3-5 years) in case management, utilization review, discharge planning and managed care
  • Professional qualities required are excellent leadership, supervisory and communication skills.

  • MSN Required and case management certification highly preferred.
  • Current licensure in PA is required.
