This position helps support the Accountable Health Community Grant.
The Connection Specialist will be responsible for helping patients and their families to navigate and access
community services and adopt healthy behaviors. The Connection Specialist acts as a liaison among hospitals,
providers, specialists, and community resources on behalf of patients to ensure coordination of care.
Community outreach, such as home visits and health screenings will may be required. The Health Advocate
will be responsible for collecting, analyzing and documenting available social services. The Health Advocate
will screen patients for unmet social needs and develop an action plan to address identified needs.

Experience and Skills
Required Skills: Must have functional knowledge in: prevalent health conditions, mental health disorders,
substance use disorders, interviewing techniques, care planning, cultural competency, self-advocacy, selfdirection,
parent/family engagement, and community specific resources, ability to assess patient need(s), and
data collection and reporting.
