Job Summary: Outpatient Clinical Documentation Specialist

Provide clinically based, daily, prospective and retrospective review of outpatient medical records.

Facilitation of appropriate provider documentation of patient care to accurately reflect the complexity of the patient’s diagnosis and treatment plan and capture of chronic conditions requiring ongoing treatment.

Works collaboratively with the Providers and Coding staff to provide physician and staff education related to clinical documentation.

Knowledgeable of Federal, State and other payer regulations and requirements that will impact documentation, reimbursement and quality measures.

Knowledge of the Medicare query guidelines.

Responsible to interact with the providers both verbally and in writing to obtain appropriate documentation clarification in the Medical Record.

Knowledge of the ICD 10 CM Coding guidelines and rules, CPT, HCC, and E/M concepts.

Strong oral and written communication skills.

Good organizational skills and good customer service skills.

Working knowledge of Microsoft Office. 

Will play a significant role in obtaining accurate and compliant reimbursement for ambulatory care services and in reporting quality outcomes



Qualifications

Education:

RN Degree required

Experience:

Relevant experience in Outpatient Clinical Documentation experience

Knowledge of the ICD 10 CM Coding guidelines and rules, CPT, HCC, and E/M concepts.

Strong oral and written communication skills.

Good organizational skills and good customer service skills.

Working knowledge of Microsoft Office.
